Dr. Nazia Rizvi, MD- is Board Certified in Internal Medicine physician with over 18 years of experience in adult primary care. She is committed to providing compassionate, personalized medical care with a strong focus on preventive health and chronic disease management.
A graduate of DOW Medical College( Class of 1994), Dr. Rizvi completed her internal Medicine residency at Trinitas Regional Medical Center in 2003. She has since been serving the community with dedication, treating a wide range of acute and chronic medical conditions.
Dr. Rizvi specializes in the management of chronic diseases such as Hypertension, Diabetes, Hyperlipidemia, Thyroid Disease, Obesity, and Asthma. She takes a comprehensive and individualized approach, helping patients understand their conditions and actively participate in their care. Through lifestyle counseling, medication management, and routine follow ups, she aims to prevent complications and improve long-term outcomes. Her goal is to empower patients to take control of their health while supporting them every step of the way.
She provides comprehensive Annual Physicals and Medicare Wellness exams to support preventive health and early detection of medical issues. In addition, she is certified to Perform Immigration Physical Examinations in accordance with USCIS requirements.

Rupalben Babaria, AGNP-C , is a dedicated and experienced Advanced Practice Nurse with a strong background in adult and geriatric primary care. She earned her Bachelor of Science in Nursing from Fairfield University and later obtained her Master of Science in Nursing with a specialization in Adult Geriatric Primary Care from Fairleigh Dickinson University. Prior to her nursing career, Rupal completed a Bachelor of Commerce in Accounting and a Master of Business Administration with a focus in Economics from South Gujarat University in India.
With over 17 years of experience in long-term care and geriatrics, Rupal is committed to delivering compassionate, patient-centered care. She excels at building strong rapport and trust with her patients, leading to improved health outcomes and high levels of patient satisfaction. Rupal is board certified as an Adult Geriatric Nurse Practitioner by the American Association of Nurse Practitioners.
Before joining Primary Health Care in November 2024, Rupal served at Central Jersey Medicine and Geriatrics , where she collaborated with primary care providers and interdisciplinary teams to co-manage high-risk patients. Her reliability, clinical expertise, and dedication make her a valuable asset to the healthcare team .
